Bring this Release of Information form if you would like to give me permission to consult with another health professional on your behalf.
For those clients wanting fast relief and are ready to be thoroughly done with certain symptoms or issues, I offer 2-day retreats in beautiful Boulder, CO.
As a Marriage and Family Therapist, I work collaboratively with Families and Multi-Generational Systems on a variety of issues and topics. Reach out if you’d like to know more about family sessions.
Unless otherwise agreed upon, payment is due at the end of the session (and prior to a phone session). I accept cash, check or credit card including HSA cards (Health Savings Account). Mobile payments received via Square or Paypal.
At this time I do not accept insurance. I can provide you with a receipt but cannot guarantee reimbursement from your insurance provider.
When considering the use of your insurance for the reimbursement of services, you need to be aware that you will be given a diagnosis. Further, if you have an insurance policy which is reviewed by one of the managed health care companies, they may want detailed information as to your personal history, drug / alcohol use, and additional information plus progress of treatment. Many times, health insurance companies may only authorize a small number of sessions averaging ten or less and couples counseling is often not covered.
